Bringing Low Vision Expertise to Your Practice

At Preventel Health, we bring unmatched firsthand knowledge of the low vision world directly to your retina practice — so your patients get the extra layer of support they need, and your practice has a true partner invested in making it happen.

Your Patients Need More Than a Clinical Visit Alone

Retina practices are doing extraordinary work — preserving sight, halting disease progression, and delivering some of the most advanced medical care available. That level of clinical excellence is exactly what your patients depend on.

But for patients living with low vision, the challenges don't end when they leave your office. They go home to a daily reality that can feel overwhelming — navigating vision loss, managing medications, adjusting to new limitations, and often doing it without the guidance or emotional support they need.

Most retina practices want to do more for these patients. What many don't realize is that there's a Medicare-approved, billable pathway to make it happen — one that brings a dedicated partner directly into your practice to deliver that support collaboratively, as a true extension of your team.

We Know This World — Because We've Lived In It

Preventel delivers best-in-class care management services across healthcare — CCM, RPM, BHI, and more. But when it comes to the retina community, it's our firsthand expertise in the low vision world that truly sets us apart from everyone else.

Partner Bari Azman grew up in a professional eye care family and spent more than a decade working directly in the low vision field, including 6½ years at a leading Mid-Atlantic low vision practice alongside his father, Dr. Thomas Azman — a pioneer with over 50 years dedicated to helping individuals with low vision reclaim their independence. That experience gave Bari something no training program can replicate: a deep, personal understanding of what low vision patients are actually living with every day.

We know the fear that follows a diagnosis. We know the isolation that can set in when independence starts to slip. We know the questions patients are afraid to ask, the burdens families quietly carry, and the moments where the right support — at the right time — can change everything.

That's the knowledge we bring to your practice. And it's why the care management we deliver doesn't feel generic to your patients — because it isn't. It comes from people who genuinely understand their world and are committed to showing up for them, every step of the way.

Our commitment to the low vision community is personal.

It's also what makes us the right partner for your practice.

The Mentor Behind the Mission

Some lessons go beyond the clinic. Bari's passion for the Low Vision Community was shaped by his father, Dr. Thomas Azman — a pioneer with 50+ years dedicated to helping individuals with low vision reclaim their independence. Working side by side for 6½ years, Bari didn't just learn the craft, he inherited the calling.

Behind every great cane is an even greater community. Bari, alongside the Awarewolf Gear family — makers of ultra-durable mobility canes built for real life — gathered for a shoot that says it all. Different faces, shared purpose. This is what supporting the Low Vision Community truly looks like.

Bari was honored to stand alongside Senator Tom Harkin — one of the driving forces behind the Americans with Disabilities Act — and Dr. Kirk Adams. A moment that connects the legislation that changed millions of lives to the community still living that mission every day.

Going the Extra Mile

When Lannie was diagnosed with stomach cancer and facing chemotherapy, Bari arranged a private jet from Baltimore to New York — not for a medical appointment, but to give him a day to breathe, laugh, and forget what was ahead. This is what it means to show up for the low vision community.

A healthcare worker dressed in black scrubs stands in a clinic hallway, looking down at a tablet she's writing on with a stylus. She wears glasses and has a badge reel and a small decorative pin on her chest. She holds a second tablet or device tucked under her arm. The background shows a blurred corridor with medical equipment.

We Become an Extension of Your Practice

Preventel doesn't operate from a distance. From day one, our team members are onsite at your practice — meeting your patients face to face, building the kind of trusted relationships that make everything else possible. That personal connection is the foundation of everything we do.

Once that relationship is established, we continue supporting your patients through ongoing remote care management — regular check-ins, medication adherence, lifestyle support, emotional guidance, family and caregiver communication, and connection to community resources. All delivered through Medicare-approved, billable services that make it sustainable for your practice to offer this level of care.

Our team works in close collaboration with yours at every step. We align with your clinical goals, communicate openly, and make sure nothing falls through the cracks between visits. Your patients experience seamless, continuous support — and your practice gains a partner who is as invested in their outcomes as you are.

Our Mission

Every retina practice has patients living with low vision who need more support than a clinical visit alone can provide. Our mission is to make sure those patients don't fall through the cracks — and that your practice has a trusted partner working right alongside you to help catch them.

We are committed to breaking down the barriers that stand between low vision patients and the care they deserve: financial, logistical, and emotional. By delivering that support through Medicare-approved, billable services, we make it sustainable for practices to offer a higher level of care — with a partner who is as invested in your patients as you are.

When your low vision patients feel supported, engaged, and confident in their care, it shows. In outcomes. In satisfaction. And in the strength of your practice.
